- Design, implement, and monitor individualized pharmacotherapy plans
- Justify recommendations using patient-specific factors and evidence-based guidelines
- Document assessments, interventions, monitoring plans, and recommendations in the patient’s medical record (HMIS)
- Adjust minor prescription elements (e.g., frequency, route) during profile review
- Design, develop, review, and update drug-related protocols
- Provide evidence-based drug information to healthcare professionals and patients
- Participate in education, training, and mentorship of pharmacists, pharmacy technicians, residents, students, and healthcare professionals
- Proactively identify, document, and report suspected Adverse Drug Reactions (ADRs) and medication errors
- Assess causality of reported events
- Identify system vulnerabilities that could lead to medication errors and suggest corrective actions
- Participate in real-time error prevention strategies
- Counsel patients on potential adverse effects of their medications
- Perform basic Root Cause Analysis

- Lead or actively participate in Antimicrobial Stewardship (AMS) initiatives
- Collaborate with physicians, nurses, and allied health professionals to optimize pharmacotherapy
- Enhance patient safety, reduce medication errors, and ensure safe Medicines Management practices
