Duties and Responsibilities:

• Ensure safe, appropriate, and cost-effective use of medicines to improve patient safety
• Plan, organize, and monitor the delivery of clinical pharmacy services at DHQ Hospital Badin
• Provide clinical pharmacy services to all medical and surgical specialties
• Participate in consultant rounds and collaborate with the multidisciplinary team
• Monitor and assess patients to identify proper therapy needs
• Improve medication compliance and coordinate with healthcare professionals in educating patients, attendants, doctors, nurses, and paramedical staff
• Implement medicine reconciliation processes upon patient admission to enhance safety
• Evaluate specific pharmacotherapy by assessing drug appropriateness, effectiveness, and identifying potential interactions or adverse effects
• Enter detailed notes on patient admission and progress in medical profiles
• Design comprehensive pharmacotherapy plans for patient-specific problems, justifying recommendations based on evidence-based information and standard practice guidelines
• Provide drug information to healthcare professionals and document accordingly
• Participate in the training of pharmacists, pharmacy technicians, and pharmacy assistants
• Maintain the hospital formulary and review non-formulary requests
• Monitor crash trolleys and floor stock on a monthly basis
• Conduct Continuing Medical Education (CME), Continuing Professional Education (CPE), and Webinars
• Prepare monthly reports on Key Performance Indicators (KPIs), interventions, expiry, near expiry, slow-moving, and dead stock
