Duties and Responsibilities:

• Ensure safe, appropriate, and cost-effective use of medicine to improve patient safety
• Plan, organize, and monitor the delivery of clinical pharmacy services at DHQ Hospital Badin
• Provide services to entire medical and surgical specialties and participate in consultant rounds
• Monitor and assess patients to identify needs and ensure proper therapy
• Improve patient compliance and coordinate with the multidisciplinary team
• Educate patients, attendants, doctors, nurses, and paramedical staff on medication use
• Perform medicine reconciliation processes upon admission to enhance patient safety
• Evaluate specific pharmacotherapy by assessing appropriateness, effectiveness, and potential interactions or adverse effects
• Enter detailed notes on patient admission and progress in medical profiles
• Design comprehensive pharmacotherapy plans based on patient-specific problems and evidence-based information
• Provide drug information to healthcare professionals and document accordingly
• Participate in the training of pharmacists, pharmacy technicians, and assistants
• Maintain the hospital formulary and review non-formulary requests
• Monitor crash trolleys and floor stock on a monthly basis
• Conduct Continuing Medical Education (CME), Continuing Professional Education (CPE), and Webinars
• Prepare monthly reports on Key Performance Indicators (KPIs), interventions, expiry, near expiry, slow-moving, and dead stock
