Clinical Pharmacy Specialist at Indus Hospital & Health Network

Skills

  • Provide comprehensive medication management for critically ill patients
  • Optimize pharmacotherapy through therapeutic drug monitoring
  • Perform medication reconciliation at ICU admission, transfer, and discharge
  • Respond to medical emergencies and time-critical events
  • Lead antimicrobial stewardship and high-alert medication management
  • Develop, implement, and evaluate evidence-based critical care protocols
  • Serve as medication-safety leader
  • Support safe, efficient, and cost-effective critical care pharmacy operations
  • Educate and mentor pharmacy students, interns, residents, pharmacists, and health professionals
  • Advance critical care pharmacy practice through research and scholarship
  • Maintain clinical competence, including Board Certification in Critical Care (BCCCP) where eligible

Duties and Responsibilities:

  • Provide expert, patient-centered pharmaceutical care to critically ill patients
  • Participate in daily rounds and prospectively review therapy
  • Perform therapeutic drug monitoring and pharmacokinetic dosing
  • Conduct medication reconciliation and provide drug information
  • Provide medication expertise during medical emergencies
  • Support appropriate selection, dosing, and monitoring of anti-infectives
  • Align critical care protocols with institutional and international standards
  • Identify, prevent, resolve, and report adverse drug events
  • Ensure medication availability and formulary-based medication use
  • Represent pharmacy on the Pharmacy & Therapeutics Committee
  • Conduct research and scholarship to advance practice
