• Establish community referrals to telehealth facilities
• Assist with tasks of related healthcare roles if required

• Travel to community homes and outreach destinations
• Collect feedback on telehealth center services
• Attend training sessions and weekly meetings
• Document qualitative and quantitative data from the field

• Travel to different program locations as needed
