• Identify stakeholders and facilitators in the communities, conduct regular meetings with them and maintain positive working relationships
• Have a good understanding of the telehealth program and how it operates
• Works collaboratively and effectively within a team
• Establish community referrals to the telehealth facility and programs
• May have to perform other tasks of RN, NA, pharmacy assistant, and receptionist in case needed
• Spread information about the telehealth center in the communities and make referrals accordingly
• Conduct health awareness sessions in the communities, homes, and in the clinic setting
• Identify community health needs and information for the core team
• Travels extensively to community homes, community locations, various NGOs, and other outreach destinations
• Collect community feedback regarding the services provided by the telehealth center
• Attend the training sessions and weekly meetings
• Document all the data coming from the field, both qualitative and quantitative, including the stories coming from the community
• Report progress and weekly updates to the program coordinator
• Performs miscellaneous job-related duties as assigned
• Support other departmental public health programs in establishing linkages with the community
