- Develop weekly and monthly outreach activity plans in coordination with the team
- Submit timely and accurate weekly, monthly, and quarterly reports on outreach activities
- Identify families with zero-dose children and ensure timely linkage with vaccination services
- Support the district coordinator in organizing health camps at the community level
- Organize monthly coordination meetings with community leaders, youth champions, government representatives, and public influencers
- Demonstrate the ability to maintain accurate record-keeping, conduct real-time surveys, and collect community feedback
- Identify, document, and submit one success story or champion family case per month to highlight community impact and promote vaccine uptake
- Collect real-time data and share monthly analysis, including people reached, feedback trends, and visual reports in the form of infographics, with the supervisor
- Must be a resident of the assigned Union Council and possess a sound understanding of local community dynamics
- Responsible for own travel within the Union Council for all field activities, including household visits, community meetings, and coordination tasks
- Perform any other task assigned by the supervisor
