Application Procedure for the Fellowship Program

Follow these steps to successfully apply for the Postgraduate Residency Program:

1. Online Application: Submit your application online through the official portal at http://level4.phf.gop.pk/.
2. Application Deadline: Ensure your application is submitted by 05:00 P.M. (PST) on December 17, 2025 (Wednesday). Note that if December 17, 2025, is declared a public holiday, the deadline will extend to the next working day until 05:00 P.M. (PST).
3. Processing Fee:
   • Pay a non-refundable processing fee of Rs. 3,000/- (Rupees three thousand only).
   • The fee must be submitted via voucher generated and downloaded from the application portal.
   • Deposit the fee at the designated bank well before the application deadline.
4. Required Documents: Prepare the following documents for submission:
   • Curriculum Vitae (CV)
   • Educational certificates (Matric, Intermediate, Bachelor's, Master's degree, etc.)
   • Professional certifications (if applicable)
   • CNIC (National Identity Card)
   • Passport-sized photographs
