2. Document Preparation: 📝
   • Prepare attested copies of the following documents:

   • Matriculation Certificate (Science Group) or equivalent
   • Intermediate Certificate (Pre-Medical) or equivalent
   • MBBS Degree or equivalent recognized by PMDC
   • Detail of Marks of all Professional Exams of MBBS
   • Attempt Certificate (MBBS) (For Local University Graduates)
   • Registration of PMDC (Valid till the last date of submission)
   • Complete House Job Certificate (01-Year)
   • Computerized National Identity Card (CNIC) (Valid)
   • Domicile (Punjab Only)
   • FCPS Part-1 Completion (Pass) Certificate/letter (Compulsory) (Only For FCPS)
   • Experience Certificate of primary, Secondary or Tertiary Healthcare Level
   • University Level Position Certificate
   • No Objection Certificate (For Government/ Semi-Government/Autonomous Body Employees)
3. Submission: 👉
   • Download the filled application form from the web-portal.
   • Submit the hard copy of the filled form along with attested copies of required documents to the office of the Medical Superintendent.
