  • Undergo CITI training for biomedical researchers, clinical research associates and good clinical practice (GCP)
  • Travel to outreach and SCC as necessary to support patient referrals and follow-up care
  • Ensure referral forms are accurately developed and readily available
  • Identify and discuss potential patients with consultants for referral to SCC or outreach center
  • Collaborate with consultants to accurately complete referral forms
  • Submit referral forms for review and approval by the consultant
  • Share referral form to the referral centre electronically on the same day of referral
  • Act as a point of contact for patients requiring referrals
  • Ensure patients understand the referral process and what to expect
  • Address patients questions or concerns regarding the referral process
  • Keep patients informed and up to date on the status of their referrals
  • Coordinate closely with consultants and healthcare professionals at Hub & SCC
  • Maintain strong working relationships with staff at outreach and SCC
  • Identify and address socio-economic problems
  • Assist the team with the collection of specific missing data for individual referred patients
  • Maintain accurate records of all patient referrals and communications
  • Ensure all referral paperwork is completed on time and in accordance with organizational guidelines
  • Assist with reporting and tracking of referral outcomes
  • Monitor referral processes to ensure they meet quality and safety standards
  • Identify opportunities for improvement in the referral process and contribute to best practices
