Duties and Responsibilities:
- Supervise and assist in out-patient Pharmacy Services inventory management
- Collect information required for interventions
- Review, monitor, and control drug profiles and CPOE of all out-patient patients
- Contact clinical pharmacists and physicians in case of order inaccuracies or medicine unavailability
- Assist clinical pharmacists in antibiotic stewardship rounds
- Assist clinical pharmacists and monitor high alert medication dispensation
- Maintain and document all interventions on the system
- Present relevant issues to clinical pharmacists and Pharmacy Services daily
- Compile and prepare presentations/data on effective interventions monthly
- Assist pharmacists in developing the hospital formulary and ensuring its implementation
- Develop strong communication between pharmacies, clinical, and nursing staff
- Identify and correct HMIS related problems in coordination with HI
- Provide summary reports to clinical pharmacists
- Review and monitor stock of all crash trolley medicines
- Present monthly reports to clinical pharmacists
- Provide current and accurate drug information to healthcare professionals
- Record drug information data and prepare graphical reports
- Provide technical and administrative support to pharmacists and pharmacy technicians
- Train interns
- Monitor and record all non-formulary drugs available at the site
- Perform all related tasks assigned by the line manager
