Application Process for FCPS Part-II Training Program

Follow these steps to apply for the FCPS Part-II Training Program at Social Security Hospital, Faisalabad:

• ✅Online Application: Submit your application through the online web-portal at www.sshmadinatownfsd.com.pk/ between December 12, 2025, and December 18, 2025.
• 📝Application Form: Download the application form from the web-portal, fill it out completely, and prepare a hard copy.
• 📑Document Submission: Submit the hard copy of the application along with duly attested copies of all required documents to the office of the Medical Superintendent.
• 🏢In-Person Submission: Deliver your application IN-PERSON to the office with all documents in an office file in the Medical Superintendent's office. Ensure you get a Diary Number upon submission. Applications sent through postal or courier services will not be entertained.
• ⏰Submission Deadline: Ensure your application reaches the Medical Superintendent's office between 08:00 AM and 02:00 PM by December 18, 2025. Applications received after the closing date will not be entertained.

• 👉 Detail of marks of all professional exams of MBBS
• 👉 Attempt certificate of professional exams
• 👉 House job certificate of 01-year completion
• 👉 FCPS Part-I completion
• 👉 Experience certificate (Primary/Secondary/Tertiary Healthcare, Rural Health Centre, etc.)
• 👉 NOC issued by competent authority (for Government/Semi-Government/Autonomous Body Employee applicants (If Any)
• 👉 University Level Position Certificate issued by competent authority (if any) certificate of position issued by principal/vice principal will not be entertained.
• 👉 Any other relevant documents.

Important Notes:

• ⏳ Candidates who resigned from a session between January 2025 and July 2025 are ineligible.
