Duties and Responsibilities:
- Identify and engage communities within the program’s catchment area
- Utilize community engagement approaches and PRA tools to build and maintain community relationships
- Identify stakeholders and facilitators, conduct regular meetings, and maintain positive working relationships
- Understand and explain the telehealth program and its operations
- Work collaboratively and effectively within a team environment
- Establish community referrals to the telehealth facility and programs
- Perform tasks of RN, NA, pharmacy assistant, and receptionist when required
- Disseminate information about the telehealth center within communities and make appropriate referrals
- Conduct health awareness sessions in communities, homes, and clinic settings
- Identify community health needs and relay information to the core team
- Travel extensively to community homes, locations, NGOs, and other outreach destinations
- Collect community feedback regarding the services provided by the telehealth center
- Attend all required training sessions and weekly meetings
- Document all incoming data from the field, both qualitative and quantitative, including community stories
- Report progress and provide weekly updates to the program coordinator
- Perform miscellaneous job-related duties as assigned
- Support other departmental public health programs in establishing community linkages
- Conduct disease screening
- Travel to different program locations (within and outside the city) as needed
