Duties and Responsibilities:
- Report on duty in time as per schedule shift and confirm assign areas
- Conduct physical check of each unit inventory, mark in checklist & report deficiencies/breakages/non-functioning to the immediate supervisor
- Ensure timely bed-to-bed handover from outgoing to incoming shift staff, including patient status, paper files, medication, investigation reports, and intake/output charts
- Document pending tasks carried forward to the incoming shift and their completion status in patient records
- Document patient take over in HMIS and any significant intervention or concern requiring urgent follow-up
- Assess patients for required care, pain, risks, and provide education on prevention (fall, infection control) in a calm and considerate manner
- Evaluate patient condition after every intervention to ascertain the effectiveness of care, medication administration, and review fall & cluster NCP in each shift
- Involve the multidisciplinary team if a patient's problem requires consultation from any specialty
- Manage the entry of general consumption of patients' disposables/supplies & medication, forward to pharmacy, and follow up until items reach the desired location
- Verify items against requisition and acknowledge receiving in HMIS as well as on the pharmacy receiving sheet
- Maintain a safe and clean working environment by ensuring compliance with housekeeping, maintenance, and safety departments
- Protect patients by adhering to IHHN infection-control & medication safe administration protocols while administering, handling multidose vials, high alert medication, and their storage
- Maintain patient privacy during hygiene care, physical examination, procedures, and transportation
- Keep patient records confidential and refrain from sharing patient data with any unauthorized healthcare worker or individuals
- Accompany physicians during rounds in the unit, note orders given, and request the doctor to write orders in HMIS regarding any change in patient care management, including medication
- Assist doctors with procedures in the procedure room/unit, prepare patients physically and emotionally, and ensure privacy & availability of necessary supplies & equipment
- Prepare patients for surgery/procedure as per pre-op orders (NPO, site preparation, chlorhexidine bath), confirm consents signed, OR dress change, blood arrangement, fresh IV line, pre-op medication administration, fresh vital signs, completion of pre-surgery checklist, and transfer on a stretcher
- Follow Post-op orders, check latest vital signs from OR, examine surgery site dressing for bleeding, cyanosis, I/V site, surgical drain/any drain, maintain I/O, and monitor for changes in vital signs/bleeding at the surgery site
- Report incidents/near misses related to any harm (patients/staff/visitors), equipment loss/breakage, or dysfunction in a timely manner with the help of NSS/area in charge, as an opportunity for improvement
- Guide IPN, RM, Nurse Aids, new joiners & Students in assigned areas for IHHN policies & practices
- Perform any other task assigned by the immediate Supervisor and/or Head of Department that is commensurate with your knowledge and skills
