- Accompany physicians during rounds and note orders
- Assist doctors with procedures, ensuring patient privacy and supply availability
- Prepare patients for surgery per pre-op orders
- Follow post-operative orders and monitor patient condition
- Report incidents and near misses promptly
- Guide LPN, RM, Nurse Aids, and students
- Perform other assigned tasks commensurate with knowledge and skills
Duties and Responsibilities:
- Report on duty in time as per schedule shift and confirm assign areas
- Conduct physical check of each unit inventory, mark in checklist & report (deficiency/breakage/ nonfunctioning) to the immediate supervisor
- Ensure timely start of bed-to-bed handover from outgoing, by incoming shift staff of the unit, patients, monitoring paper files, medication, investigation reports, intake & output chart etc
- & their documentation
- Discuss pending tasks carry forward to incoming shift and its documentation in patient record for not been able to complete
- Document take over in HMIS and any significant intervention or concern to follow urgently
- Assess patients for required care, pain assessment, risks & education on prevention (fall, infection control in a calm/considerate manner
- Evaluate patient condition after every intervention to know the effectiveness of given care, medication administration and review fall & cluster NCP in each shift
- Involve multidisciplinary team if patient problem need consultation from any specialty
- Manage general consumption of patients' (disposables/supplies & medication), forward to pharmacy, follow until items reach at desired location, verify against requisition & then acknowledge receiving in HMIS as well as on pharmacy receiving sheet
- Maintain safe and clean working environment by ensuring compliance with housekeeping, maintenance and safety departments
- Protect patients by adhering with IHHN infection-control & medication safe administration while administering, handling multidose vials, high alert medication and their storage
- Maintain privacy of patient during giving hygiene care, physical examination, any procedures & transportation
- Keep confidentiality of patients record and don't sharing patient data with any unauthorized HCW/individuals
- Accompany physicians' during rounds in the unit, note order given during round & request Dr
- To write order in HMIS about any change in patient care management including medication
- Assist doctors with procedure they need to perform in the procedure room/Unit if no procedure room, prepare patient physically, as well as emotionally and ensure privacy & availability of necessary supplies & equipment
- Prepare patient for surgery/procedures per pre-op order (NPO, site preparation, chlorhexidine bath & confirm (consents signed, change of OR dress, blood arrangement, fresh IV line, pre-op medication administration if any, fresh vital signs) completion of pre surgery checklist and transfer of on stretcher
- Follow Post op order, check latest vital signs from OR, check surgery site dressing for bleeding, Cyanosis, I/V site, surgical drain/any drain, maintain 1/0, and be watchful for change in vital signs/bleeding at surgery site
- Report incident/near miss reporting related to any harm (patients/staff/visitors, equipment loss/breakage or dysfunction in a timely manner with the help of NSS/area in charge, as an opportunity
- Guide to LPN, RM, Nurse Aids, new joiner & Students in assigned areas for IHHN policies & practices
- Perform any other task assigned by the immediate Supervisor and/or Head of Department that is commensurate with your knowledge and skills
