Duties and Responsibilities:

• Identify and liaise with community stakeholders and facilitators
• Document qualitative and quantitative data from the field, including community stories
• Provide weekly progress reports and updates to the program coordinator
• Identify and register target populations for health programs
• Educate families and caregivers on the importance of routine immunization
• Mobilize community members for immunization sessions
• Assist vaccinators during outreach sessions, including crowd management
• Maintain accurate household and immunization records
• Follow up with families to ensure vaccination completion
• Support cold chain equipment maintenance during sessions if needed
• Report any adverse events following immunization (AEFI) promptly
• Undertake any other assigned tasks by the supervisor
