Identify community stakeholders and facilitators, conducting regular meetings
Understand and explain the telehealth program's operations
Work collaboratively and effectively within a team environment
Establish community referrals to the telehealth facility and programs
Perform tasks of RN, NA, pharmacy assistant, and receptionist as needed
Spread information about the telehealth center and make referrals
Conduct health awareness sessions in communities, homes, and clinics
Identify community health needs and provide information to the core team
Travel extensively to community homes, locations, NGOs, and outreach destinations
Collect community feedback on telehealth services
Attend training sessions and weekly meetings
Document all field data, including qualitative and quantitative information and community stories
Report progress and weekly updates to the program coordinator
Perform miscellaneous job-related duties as assigned
Support other departmental public health programs in establishing community linkages
Conduct disease screening
Travel to different program locations (within and outside the city) when needed
