Duties and Responsibilities:

• Identify and engage communities within the program’s catchment area
• Utilize community engagement approaches and PRA tools to build strong community relationships
• Identify and maintain positive working relationships with community stakeholders and facilitators
• Develop a good understanding of the telehealth program's operations
• Work collaboratively and effectively within a team environment
• Establish community referrals to the telehealth facility and programs
• Perform tasks of RN, NA, pharmacy assistant, and receptionist if needed
• Spread information about the telehealth center and make appropriate referrals
• Conduct health awareness sessions in communities, homes, and clinics
• Identify community health needs and relay information to the core team
• Travel extensively to community homes, locations, NGOs, and outreach destinations
• Collect community feedback regarding telehealth center services
• Attend training sessions and weekly meetings
• Document all field data, both qualitative and quantitative, including community stories
• Report progress and weekly updates to the program coordinator
• Perform miscellaneous job-related duties as assigned
• Support other departmental public health programs in establishing community linkages
• Assist with disease screening
• Willing to travel to different program locations when needed
