Spread information about the telehealth center in the communities and make referrals

Identify community health needs and information for the core team
Travel extensively to community homes, locations, NGOs, and other outreach destinations
Collect community feedback regarding the services provided by the telehealth center
Attend training sessions and weekly meetings
Document all data coming from the field, both qualitative and quantitative, including stories from the community
Report progress and weekly updates to the program coordinator
Perform miscellaneous job-related duties as assigned
Support other departmental public health programs in establishing linkages with the community
Conduct disease screening
Willing to travel to different program locations (within and outside the city) when needed
