Duties and Responsibilities:
- Engage communities within the program’s catchment area
- Utilize community engagement approaches and PRA tools
- Develop and maintain strong relationships with community stakeholders
- Identify stakeholders and facilitators in communities
- Conduct regular meetings with stakeholders
- Establish community referrals to the telehealth facility and programs
- Perform tasks of RN, NA, pharmacy assistant, and receptionist if needed
- Spread information about the telehealth center in communities
- Conduct health awareness sessions in communities, homes, and clinics
- Identify community health needs and provide information to the core team
- Travel extensively to community homes, locations, NGOs, and outreach destinations
- Collect community feedback regarding telehealth center services
- Attend training sessions and weekly meetings
- Document all data from the field, including qualitative and quantitative information
- Report progress and weekly updates to the program coordinator
- Perform miscellaneous job-related duties as assigned
- Support other departmental public health programs in establishing community linkages
- Conduct disease screening
- Travel to different program locations when needed
