Duties and Responsibilities:

• Identify and engage communities within the program's catchment area
• Utilize community engagement approaches and PRA tools to build strong community relationships
• Identify and liaise with community stakeholders and facilitators
• Develop a thorough understanding of the telehealth program's functions
• Work effectively and collaboratively within a team environment
• Establish community referrals to the telehealth facility and programs
• Perform duties of RN, NA, pharmacy assistant, and receptionist if required
• Disseminate information about the telehealth center and make appropriate referrals
• Conduct health awareness sessions in various settings
• Identify community health needs and relay information to the core team
• Travel extensively to community homes, NGOs, and outreach destinations
• Collect feedback from the community regarding telehealth services
• Attend training sessions and weekly meetings
• Document qualitative and quantitative data from the field, including community stories
• Report progress and weekly updates to the program coordinator
• Perform miscellaneous job-related duties as assigned
• Support other public health programs in establishing community linkages
• Conduct disease screening
• Willingness to travel to different program locations as needed
