• Ability to coordinate with floor Doctors and Staff
• Skills in patient Pharmacy Services inventory management
• Capability to collect information for interventions
• Experience in reviewing and monitoring drug profiles
• Competence in Computerized Physician’s Order Entry (CPOE)
• Ability to contact physicians for order inaccuracies or medicine availability
• Participation in Antibiotic stewardship rounds
• Monitoring of high alert medication dispensation
• Documentation of all interventions on the system
• Preparation of monthly presentations on effective interventions
• Assisting in hospital formulary development and implementation
• Developing strong communication between Pharmacies and floor staff
• Identification and correction of HMIS related problems
• Review and monitoring of crash trolley medicine stock
• Providing current and accurate drug information
• Recording drug information data and preparing graphical reports
• Providing technical and administrative support to Pharmacists and Technicians
• Training of interns
• Counseling Doctors, staff, patients, and families on safe drug use
• Arranging Continuing Medical Education (CME) programs
• Monitoring and recording non-formulary drugs
• Performing assigned tasks by Pharmacy management

Duties and Responsibilities:

• Participate in clinical rounds and coordinate with medical staff
• Supervise and assist in pharmacy inventory management
• Review and control drug profiles for all admitted patients
• Contact physicians regarding medication orders and availability
• Contribute to antibiotic stewardship initiatives
• Monitor high alert medications and provide necessary information
• Document all pharmacy interventions and report daily issues
• Prepare monthly presentations on intervention effectiveness
• Support the development and implementation of the hospital formulary
• Enhance communication between pharmacy and floor staff
• Address and resolve HMIS issues in coordination with IT
• Monitor and report on crash trolley medicine stock monthly
• Provide comprehensive drug information to healthcare professionals
• Record and report monthly drug information data
• Offer technical and administrative support to pharmacy staff
• Train pharmacy interns
• Counsel patients and families on medication usage
• Organize CME programs related to rational drug use
• Track and record all non-formulary drugs for inpatients
• Execute all tasks assigned by Pharmacy management
