Duties and Responsibilities:

• Identify and engage communities within the program's catchment area
• Develop and maintain strong relationships with community members and stakeholders
• Utilize community engagement approaches and PRA tools
• Establish community referrals to the telehealth facility and programs
• Spread information about the telehealth center and make referrals
• Conduct health awareness sessions in communities, homes, and clinics
• Identify community health needs and relay information to the core team
• Travel extensively to community homes, locations, NGOs, and outreach destinations
• Collect community feedback on telehealth services
• Attend training sessions and weekly meetings
• Document qualitative and quantitative data from the field
• Report progress and weekly updates to the program coordinator
• Perform miscellaneous job-related duties as assigned
• Support other public health programs in establishing community linkages
• Assist with disease screening
• Willing to travel to different program locations as needed
