Duties and Responsibilities:

• Identify and engage communities within the program’s catchment area
• Utilize community engagement approaches and PRA tools to develop and maintain strong community relationships
• Identify stakeholders and facilitators in communities, conduct regular meetings, and maintain positive working relationships
• Develop a good understanding of the telehealth program and its operations
• Work collaboratively and effectively within a team
• Establish community referrals to the telehealth facility and programs
• Perform other tasks of RN, NA, pharmacy assistant, and receptionist in case needed
• Spread information about the telehealth center in communities and make referrals accordingly
• Conduct health awareness sessions in communities, homes, and the clinic setting
• Identify community health needs and information for the core team
• Travel extensively to community homes, community locations, various NGOs, and other outreach destinations
• Collect community feedback regarding the services provided by the telehealth center
• Attend training sessions and weekly meetings
• Document all data coming from the field, both qualitative and quantitative, including stories from the community
• Report progress and weekly updates to the program coordinator
• Perform miscellaneous job-related duties as assigned
• Support other departmental public health programs in establishing linkages with the community
• Conduct disease screening
• Willing to travel to different program locations (within and outside the city) when needed
