Duties and Responsibilities:

• Develop weekly and monthly outreach activity plans in coordination with the team
• Submit timely and accurate weekly, monthly, and quarterly reports on outreach activities
• Identify families with zero-dose children and ensure timely linkage with vaccination services
• Support the district coordinator in organizing health camps at the community level
• Organize monthly coordination meetings with community leaders, youth champions, government representatives, and public influencers
• Maintain accurate record-keeping, conduct real-time surveys, and collect community feedback
• Identify, document, and submit one success story or champion family case per month
• Collect real-time data and share monthly analysis with the supervisor
• Undertake own travel within the Union Council for all field activities
• Perform any other task assigned by the supervisor
