• Conducting door-to-door household visits
• Organizing and facilitating community awareness sessions
• Screening, referral, and follow-up of ANC, PNC, and EPI due and missed cases
• Referring eligible beneficiaries to Mobile Health Teams and Facilitation Centers
• Coordinating with LHWs, teachers, Union Council Chairmen, religious leaders, and community elders
• Maintaining daily activity logs, beneficiary lists, and reports
• Supporting monitoring, evaluation, and donor reporting

• Development of taluka-specific action plans
• Mobilizing due-follow up cases for timely assistance
• Addressing compliance issues and facilitating documentation processes (CNIC, B-Form)
• Resolving documentation issues for beneficiaries

Duties and Responsibilities:

• Conducting door-to-door household visits to assess health needs
• Organizing and facilitating community awareness sessions and school-based sessions on health topics
• Screening, referring, and following up on antenatal care (ANC), postnatal care (PNC), and immunization (EPI) due and missed cases
• Referring eligible beneficiaries to Mobile Health Teams and Facilitation Centers (FCs) for further care
• Coordinating effectively with Lady Health Workers (LHWs), teachers, Union Council Chairmen, religious leaders, and community elders
• Maintaining accurate daily activity logs, beneficiary lists, and comprehensive reports
• Supporting monitoring, evaluation, and donor reporting activities
• Identifying malnutrition cases and establishing appropriate referral pathways
• Identifying key stakeholders and engaging them in community health initiatives
• Developing taluka-specific action plans to address local health challenges
• Mobilizing beneficiaries, particularly those in red and yellow categories, for timely assistance and follow-up
• Resolving compliance issues and facilitating beneficiaries with documentation such as CNIC and B-Form
