  • Utilize community engagement approaches and PRA tools to build and maintain community relationships
  • Identify community stakeholders and facilitators, conducting regular meetings
  • Understand and explain the telehealth program's operations
  • Work collaboratively and effectively within a team environment
  • Establish community referrals to the telehealth facility and programs
  • Perform tasks of RN, NA, pharmacy assistant, and receptionist as needed
  • Spread information about the telehealth center and make referrals
  • Conduct health awareness sessions in communities, homes, and clinics
  • Identify community health needs and provide information to the core team
  • Travel extensively to community homes, locations, NGOs, and outreach destinations
  • Collect community feedback on telehealth services
  • Attend training sessions and weekly meetings
  • Document all field data, including qualitative and quantitative information and community stories
  • Report progress and weekly updates to the program coordinator
  • Perform miscellaneous job-related duties as assigned
  • Support other departmental public health programs in establishing community linkages
  • Conduct disease screening
