Duties and Responsibilities:
- Report on duty in time as per schedule shift and confirm assigned areas
- Conduct physical check of each unit inventory, mark in checklist & report deficiencies/breakages/non-functioning items to the immediate supervisor
- Ensure timely start of bed-to-bed handover from outgoing to incoming shift staff, including patient status, monitoring paper files, medication, investigation reports, and intake/output charts
- Document handover details in HMIS and any significant interventions or concerns requiring urgent follow-up
- Assess patients for required care, pain, risks, and provide education on prevention (fall, infection control) in a calm and considerate manner
- Evaluate patient condition after every intervention to determine the effectiveness of care, medication administration, and review fall & cluster NCP in each shift
- Involve the multidisciplinary team if patient problems require consultation from any specialty
- Manage the entry of general consumption, patient supplies (disposables/supplies & medication), forward to pharmacy, and verify against requisition
- Maintain a safe and clean working environment by ensuring compliance with housekeeping, maintenance, and safety departments
- Protect patients by adhering to IHHN infection-control and medication safe administration protocols
- Maintain patient privacy during hygiene care, physical examination, procedures, and transportation
- Keep patient records confidential and refrain from sharing patient data with unauthorized individuals
- Accompany physicians during rounds, note orders, and request Dr
- To write orders in HMIS for any changes in patient care management
- Assist doctors with procedures, prepare patients physically and emotionally, ensure privacy, and availability of necessary supplies and equipment
- Prepare patients for surgery/procedures per pre-op orders, including consent confirmation, site preparation, and completion of pre-surgery checklists
- Follow post-operative orders, check vital signs, surgical site dressing, and monitor for changes
- Report incidents/near misses related to patient/staff harm, equipment loss/breakage in a timely manner
- Guide LPN, RM, Nurse Aids, new joiners, and students in assigned areas regarding IHHN policies and practices
- Perform any other task assigned by the immediate Supervisor and/or Head of Department
